Our August In-Service will include presenters from Imagine!, a program which serves children with disabilities, and from the programs below:
The Children with Special Needs program at Boulder County Public Health can provide outreach education and resource support to you as it relates to infants/children/teens you may see who have developmental delays or significant health conditions. Our registered nurses provide short term care coordination to strengthen families to navigate the systems that support their child’s health and development. Our program provides support to the families of children birth to 21 years of age. In addition to direct care coordination we are also the Boulder County resource center for special needs and can provide support to community partners via information on special needs resources.
The Boulder County Assuring Better Child Health and Development Partnership works to improve the developmental screening process in our community. Through our outreach events we support community understanding of whole child development and the developmental screening process (developmental monitoring, screening, referral, evaluation, service delivery). An emphasis is placed on how to refer for evaluation. Our end goal is that children will receive the supports they need to enter the school setting as confident, healthy learners. We provide education and materials to support our community partners to make effective referrals.
